By checking this box I the customer with regard to Fast Specialties and or its service department, their principles, agents, or assigns, herein after referred to as the agent, it is understood that:

1. Customer desires to have agent operate his/her vehicle for testing and evaluation.
2. Customer grants permission for agent to operate his/her vehicle in any manner it deems necessary, including on or off a chassis dynamometer. Customer assumes full responsibility for loss or damage during said operation.
3. Customer agrees that the agent is not responsible for any loss or damage to his/ her vehicle of persons as a result of the operation of said chassis dynamometer. Agent has made no representation other than those expressly acknowledged and understands the following:
   1. Vehicles can suffer serious damage or after damages on a chassis dynamometer test and many of its mechanical components, occasionally, to critical levels which by its very nature may cause failure to those components.
   2. Automobiles are inherently unpredictable and unsafe, and operation or riding in a vehicle or being in the vicinity of a vehicle being operated or tested on a chassis dynamometer can lead to serious injury or death.

4.  Customer agrees that all risk of loss or damage to his/ her vehicle or person with regard to operating testing of his/her vehicle, remains with him/her.

IN SHORT, THE CUSTOMER IS RESPONSIBLE FOR EVERYTHING INCLUDING ANY DETERMINATION OF ROADWORTHINESS OR SAFETY OF HIS/HER VEHICLE AFTER OPERATION BY AGENT, AND EXPRESSLY ACKNOWLEDGES THAT THE AGENT IS NOT RESPONSIBLE FOR ANY LOSS OR DAMAGE TO HIS/HER VEHICLE OR PERSONS RESULT OF THIS OPERATION OR TESTING AND HEREBY RELEASES, DISCHARGES, AND INDEMNIFIES AGENT OR ANY AND ALL CAUSES OF ACTION ARISING FROM OR AS A RESULT OF THE OPERATION OF HIS/HER VEHICLE BY AGENT.

I have read the foregoing document in its entirety and understand the terms and condition contained herein. This release in continuing in nature and shall apply to this and any other vehicle I own or have control over which I deliver to the agent. If I desire to terminate this release I will do so in writing, and will deliver said termination to agent via certified US Mail, with a return receipt required.
